Dear Colleagues,
Major changes have occurred in understanding and treating multiple sclerosis in the last 25 years. New diagnostic criteria will be available soon and an interesting discussion is ongoing on the classification of the courses. Early treatment and individualized approach are the key aspect of the therapy with more attention also to improve recovery and adaptation to the damage with a growing role of rehabilitation. Still critical a better knowledge of mechanisms underlying progression in order to a better treatment for progressive multiple sclerosis.
Given the major advances in the treatment of Multiple Sclerosis, it is now time to re-think about how to better use the available disease modifying treatments, when and how to start the treatment, the sequence of treatments, when to discontinue therapy. There is also quite relevant evolution for the diagnosis, as new diagnostic criteria will be delivered soon and the concept of silent progression and progression independent from relapse activity have promoted a revision of the classification of the disease courses and revealed the importance of the pathophysiological mechanisms underlying progression. The growing evidence on the importance of rehabilitation and new modalities of intervention, such as neuromodulation, may have a relevant impact on the quality of life of person with multiple sclerosis and need a larger implementation. Individualized medicine requires a careful definition of the disease trajectory and more precise monitoring of the response of treatment, with a special role for digital medicine. We still miss a really effective treatment for progressive multiple sclerosis because of the relatively modest knowledge on the role of the various pathophysiological mechanisms. Of great importance the availability of therapeutic approaches to enhance recovery.
Papers addressing these key aspects are very welcome.
